POSTER PRESENTATIONS

Local health districts, Specialty health networks, Pillars and Shared services are invited to showcase their achievements or innovative approaches by poster presentation at the Value based healthcare forum.

Up to two posters can be submitted by each district or network. You are welcome to use the same content being developed for the Value based healthcare category in the upcoming NSW Health Awards.

Posters must include the following headings and be no longer than 350 words in length. Poster size to be A1, or the equivalent of 8 A4 sheets.

Concurrent Poster Stream: Choose the most relevant theme from 1 to 18 in the table below 

Abstract Title: No more than a short sentence required

Authors / Presenters: Authors' first and last name with presenting author in bold. Primary position title (one only) and organisation included. Do not include degrees or proefessioanl titles (e.g. Dr, Prof, etc.)

Background: Describe the identified area for change and its relevance to healthcare in your setting. Spell out acronyms in full the first time they appear (e.g. Leading Better Value Care (LBVC))

Approach: What Methods / strategies were used to address the problem?

Outcomes / Results Provide evidence of the impact of your initiative. What was achieved and how was that measured?

Take Home Message: What did you learn, what was your experience? What should others know?


POSTER THEMES:
The theme for posters focusses on the lessons learned and opportunities for scaling and sustaining the Leading Better Value Care (LBVC) clinical initiatives and non-clinical initiatives.

1) Chronic heart failure
2) Chronic obstructive pulmonary disease
3) Inpatient management of diabetes mellitus
4) Falls in hospital
5) High risk foot services
6) Osteoarthritis chronic care program
7) Osteoporosis re-fracture prevention
8) Renal supportive care
9) Hip fracture
  10) Bronchiolitis
11) Direct access colonoscopy
12) Wound management
13) Hypofractionated radiotherapy for early stage breast cancer
14) Non-clinical LBVC initiatives - e.g. workforce, shared services
15) Program level (including innovative local processes for change)
16) LHD system based approaches including using data for better patient outcomes
17) Used a VBHC approach for non-LBVC initiative
18) The development of strategic partnerships that have resulted in better patient care outcomes


PRESENTATION: Posters will be showcased on both days of the forum.
In addition to being available to interested delegates in the morning and lunch breaks of days 1 and 2, authors will be provided the opportunity to speak to their posters in a poster presentation session on the afternoon of Day 2 of the forum.


PRIZES: A prize will be awarded for the best poster as judged by a panel. A People’s Choice Award will also be made.

SUBMISSIONS: Please advise Isabella Mee (Director, Conference and Event Manager) of the title and authors of the poster by COB Friday 2 August. Posters will be included in the conference booklet.
